Smart Excuses:
I've earned it: Usually employers do not argue with performance. If you are working hard for several days or weeks in a row, then negotiate a day off in advance.
Doctor's appointment: Usually make your appointment in the afternoon (3pm) or in the morning. This is a pretty legitimate excuse for missing work and one that is not challenged (unless of course you have a lot of Doctors appointment they may start asking for a note).
Meeting clients: if you are into sales, marketing, advertising, servicing, this can be a good excuse. However, this will take some strategic coordinating. Maybe have an appointment with a client then take the rest of the day off. This can back fire if you are not careful.
Cramp: Employers typically do not question this one as it is a sensitive topic. It's pretty embarrassing for men to talk about it and women will usually empathize.
Working from home: Use this one only if you have a job that allows you work from home. Get some rest, but make sure you do some more work at home or else your boss will never believe you again.
